Physical restraints + seclusion rooms (pre-moral-treatment remnants)
The dominant "tools" of early asylum attendants were physical: leather restraints, iron manacles, locked seclusion rooms, cold-water baths, and mechanical chair restraints. These were not therapeutic innovations but inherited instruments of custodial control dating to the pre-asylum era. The American moral treatment movement of the 1840s-1860s, championed by Dorothea Dix and Samuel Woodward at Worcester State, tried to replace physical restraint with structured environment and occupation therapy, and achieved genuine reform in some institutions. But the census explosion of the 1870s-1880s (immigration-driven, combined with a broadening of commitment criteria) overwhelmed moral-treatment staffing ratios. By 1890, most large state hospitals had returned to routine restraint use. The attendant's toolkit was custodial, not clinical.

Chlorpromazine (trade name Thorazine) was introduced to US psychiatric hospitals in 1954 after French surgeons observed its calming effect in anesthesia. Within two years, chlorpromazine had transformed the work of every attendant on every ward where it was used. Patients who had previously required constant physical restraint could now be managed with medication, and the chronic agitation that defined many wards subsided dramatically. The attendant's role shifted from physical control toward observation and medication administration. Chlorpromazine and the antipsychotics that followed also provided the pharmacological justification for deinstitutionalization: if patients could be stabilized on medication, the argument went, they could be managed in the community. The promise was genuine but the community infrastructure never matched the pace of hospital discharge.

For psychiatric technicians, the transition meant learning to document 15-minute patient observations, medication passes, and safety incidents in a structured EHR rather than on paper -- a significant workflow change that added a computing component to a role previously defined entirely by physical presence.
