Manual care with no instruments (pre-professionalization era)
The hospital attendant of the Civil War and early-hospital era had no instruments of their own. The work was entirely physical and observational: changing linen, bringing food and water, repositioning bedridden patients, cleaning and dressing wounds under a nurse or surgeon's direction. The ward orderly's tools were a bedpan, a basin, and their hands. The only technology that entered the ward was shared with the nursing staff: the clinical thermometer (introduced to US practice after the Civil War, widespread by 1880), the hypodermic syringe (1860s), and, by the early twentieth century, the basic vital-signs kit. The attendant did not use these instruments; they were the domain of the nurse. What the attendant provided was physical labor and presence, and that function has not changed in 160 years.

By the postwar era, nursing aides had taken on the routine vital-signs tasks that occupied a growing share of basic patient monitoring: temperature, blood pressure (the sphygmomanometer became standard ward equipment in the 1940s), pulse and respiration, and simple intake-output measurement. The bedside call light system, which became standard in hospital ward design after World War II, gave aides a direct communication line with patients and created the demand-response workflow that remains the structural spine of the nursing assistant's day. These tools augmented the aide's reach but did not change the fundamentally physical nature of the role: bathing, feeding, transferring, and toileting patients remained entirely manual, and the physical demands of the job placed nursing aides among the occupations with the highest rates of musculoskeletal injury.

The Omnibus Budget Reconciliation Act of 1987 mandated minimum 75-hour training and state competency examinations for all nurse aides in Medicare/Medicaid-certified facilities, effective January 1, 1990. This transformed the nursing aide from an informal labor category into a credentialed occupation: the Certified Nursing Assistant. State registries tracked who held a valid CNA credential, and facilities faced federal penalties for using unregistered staff. The training standardized basic clinical skills (vital signs, catheter care, range-of-motion exercises, body mechanics) that had previously varied wildly by facility. Simultaneously, powered patient lift devices began entering nursing homes in the 1990s as facilities responded to alarming injury-rates among staff performing manual transfers. Mechanical lifts reduced back injuries but added equipment-operation skills to the CNA's role.
Effect on the workOBRA 1987 certification increased the formal skill bar for entry-level CNAs and may have slightly reduced the supply of casual labor willing to obtain the credential, but the credential itself was not particularly restrictive (75 hours of training). The main labor effect was institutional: facilities could no longer legally staff wards with uncredentialed workers, which nudged wages modestly upward in the early 1990s.

The rollout of EHR systems across US hospitals and skilled nursing facilities in the 2005-2015 period reached nursing assistants in a specific form: mobile nursing carts and point-of-care terminals that required CNAs to document vital signs and activities of daily living digitally rather than on paper flow sheets. Epic's bedsidePOC module and MEDITECH's nursing documentation interface became standard in hospital settings. For nursing assistants, EHR adoption meant a new required skill (basic computer literacy, navigating the documentation interface) layered on top of the existing care tasks. The transition was often poorly supported: facilities frequently lacked adequate EHR training for nursing assistants, who were expected to adapt alongside RNs despite having less familiarity with digital tools. The upside was that digitally documented vitals and ADL data fed directly into the nurse's clinical view, closing a communication gap that paper flow sheets had left open.

Two AI-powered tools reached nursing assistant workflows in the 2020s. First, ambient documentation: Commure Ambient for Nursing (launched November 2025 inside Epic Rover) and Aiva Nurse Assistant captured bedside conversations and auto-populated EHR flowsheets, removing the keystroke burden from documentation. A Cedars-Sinai pilot of Aiva with 120+ nurses showed an 81% reduction in time between patient observation and documentation completion. Second, sensor-based fall prevention: VirtuSense VSTAlert, deployed in skilled nursing facilities from around 2018 onward, uses infrared sensors to detect bed-exit intent 31-65 seconds before the resident actually rises, alerting staff with 98% accuracy and 95% fewer false alarms than traditional bed alarms. Facilities using VSTAlert reported 75-85% reductions in falls. Both tools augment rather than displace the nursing assistant: the documentation AI reduces the time spent at terminals, and the fall sensor extends the aide's effective span of attention across a multi-patient assignment. Neither replaces the hands that bathe, feed, or transfer a patient.
Effect on the workNo direct displacement of CNA positions has been documented from either tool type. Ambient documentation frees approximately 15-20 minutes per shift from documentation tasks; fall-prevention sensors allow one CNA to effectively monitor more residents without increased fall rates. McKinsey estimates approximately 10% of nursing activities could be freed by automation, almost entirely from the documentation and information-relay portion of the job.

Licensed Practical and Licensed Vocational Nurses
Licensed Practical Nurses perform many of the same hands-on care tasks as CNAs but with expanded clinical authority (medication administration, wound care, IV maintenance) and higher median pay ($59,730 vs $39,530). The path from CNA to LPN typically requires 12-18 months of accredited education plus passing the NCLEX-PN; facilities often offer tuition assistance for CNAs who commit to the path.
